Five Wishes by Nancy Ludin, Jewish Pavilion CEO

The Five Wishes program was developed as the first advance care plan to address personal, emotional, and spiritual wishes, in addition to medical treatment. It’s called the “living will with heart and soul” because it’s based on what is most important – being able to define a roadmap for how you want to be cared for. It has been embraced by families, caregivers, community groups, faith communities, medical and legal providers, and businesses.
40 Million Copies have been distributed over 25 years,.  There has been directives and positive feedback from individuals, families and caregivers, as well as medical providers, The document has helped them feel more at ease knowing that their loved one's wishes were detailed and in a legally valid document. 
While most seniors face major adjustments when transitioning to an elder-care community, Jewish seniors face additional challenges. Not only do they lose their homes, and many of their friends, but they also lose ties to their cultural heritage.  This is where the Jewish Pavilion, a 501c3 non-profit, steps in.  The Pavilion serves as a resource that provides room visits, festive holiday celebrations, and more to 450 Jewish residents in fifty facilities for seniors.  The Jewish Pavilion promotes inclusion, and thousands of seniors of all faiths are welcomed into our programs. www.JewishPavilion.org
